Hours: 37 hours per week

Location: Civic Centre / Longton Cottage Hospital

We are looking for an Occupational Therapist Post based in a hospital social work team covering discharges from acute hospitals and intermediate settings.  In this position you will need to be able to work in a fast paced environment and be an integral part of the team. You will ensure a smooth transition for Stoke citizens from hospital discharge pathways for adults who experience illness, are disabled or have care and support needs. Supporting citizens with advice and information, as well as, through an assessment of their needs and assist in the support planning of their care.  

Your normal hours of work are based on an average of 37 hours per week, working on a rota basis in connection with the seven day opening of the Hospital Team.
