There are a number of Social Worker roles we are recruiting to across the Older People’s Assessment and Liaison Team (OPAL) based in hospitals. It is an exciting time to be part of this service as Birmingham continues to develop the whole system Discharge to Assess (D2A) model with system partners within the Early Intervention Programme.

The Early Intervention programme has been developed between health and social care partners across Birmingham with the aim of transforming the lives of thousands of people for years to come. The system partners include Birmingham City Council, Birmingham Community Healthcare Trust, University Hospitals Birmingham, Birmingham & Solihull Mental Health Trust, Birmingham & Solihull CCG and Sandwell and Birmingham CCG. Early Intervention provides urgent assessment, treatment and care to people; as well as a range of integrated services that promote recovery and independence.

All health and social care professionals working side-by-side in multi-disciplinary teams support the ‘home first’ ethos, in which people - patients and residents - receive the right care, at the right time, in the right place, by the right professional, at the right cost.

This is an exciting opportunity to become part of a dynamic multi-disciplinary team working in a front-line hospital position. There are roles available in the OPAL teams within Birmingham Heartlands Hospital, Good Hope Hospital and Queen Elizabeth Hospital. Please note, this is a 7-day service so includes some weekend work which will be based on a rota.

Working to achieve the Early Intervention vision, OPAL is a multi-disciplinary team which includes doctors, nurses, therapists, social workers and pharmacists, amongst many others. The teams see older people as soon as they arrive and liaise closely with community services to enable a "home-first" approach.

The OPAL teams work to the 3 Conversations model in principle, mainly undertaking conversation 1 type activity and supporting referrals on to appropriate services. This role includes diverting to community services and thinking creatively to support people in their own homes. The post holders will link with the other hospital and out of hospital teams including the Early Intervention Community Team to provide support where needed in the community.

Within this role, Social Workers will be considering identified risks, working in partnership with people in regard to their managing their needs and working within the safeguarding process where required.

Professional Social Work qualification and registration with Social Work England is essential.

The post holder will have experience of assessing under the Care Act 2014, understand and have a working knowledge of safeguarding processes and procedures and be able to think creatively within a multi-disciplinary setting.
