£33,782 - £38,890 per annum
This is an exciting time to join Walsall Adult Intermediate Care Services.
We are a dynamic, welcoming team working with adults, their carer’s and families. We support people from age 18 years onwards and work with individuals to build their confidence, and develop their skills following a hospital admission.
The Integrated team is made up of a Service Manager, Team Manager, 4 Advanced Practitioners, Social Workers, Social Care Facilitators, Occupational Therapist, Physiotherapy, Nurses and Rapid Response. We undertake assessments, support planning and reviews under the Care Act 2014, working closely with Health partners, Commissioners and many external agencies in order to provide an integrated approach to supporting individuals.
We currently have 4 permanent full time vacancies for experienced Social Workers to join our team. We are looking for an enthusiastic, proactive and committed professionals to join our team, to help shape and deliver our service, and contribute to the development of best practice.
The successful applicants will be skilled in carrying out strengths based assessments and support planning under the Care Act 2014, completing comprehensive risk assessments, Mental Capacity assessments and Best Interest Decision Making. You are required to have experience of working within a multi-disciplinary setting.
We are looking for a forward thinking team members who will promote and deliver positive outcomes for individuals and their carers. Therefore, you will need to demonstrate significant experience of working with vulnerable people leaving hospital. Integral to this role is having a comprehensive working knowledge and understanding of Making Safeguarding Personal and experience of undertaking safeguarding enquiries.
