Salary:  £37,890- £41,881 per annum

 37 Hours per week – Secondment

We are seeking to recruit 1 Advanced Practitioner to work within the First Contact Adult Social Care Team.

This key role will take responsibility for the working across Older People, Physical and Learning Disabilities, Hospital Discharge referrals via Home First and all age Carers assessments, as such we are seeking experienced Advanced Practitioners with a range of knowledge and skills across this broad agenda.

 Key responsibilities will include supervising and supporting the professional development of social workers and social care assessors. To manage unpredictable daily changes to work pattern, using initiative to organise daily duties whilst adhering to SCC policy and procedure to meet service demands, seeking guidance from operational managers when required. To be responsible for the Quality assurance of statutory documents and promoting and embed the ethos of “Just enough Support”.

If you have a desire to support the Local authority in developing our approach to supporting citizens at the first point of contact, this role could be for you.

This position is subject to a criminal records check from the Disclosure and Barring Service (formerly CRB) which will require you to disclose details of all unspent and unfiltered spent reprimands, formal warnings, cautions and convictions in your application form.

This position will have access to the Public Sector Network, to meet government security standards we will request a basic criminal convictions certificate and other background checks prior to appointment for the successful candidate
